How LFT and blood reports look like in Alcoholic liver disease

1. Liver Function Test (LFT)

A) AST (SGOT) and ALT (SGPT) are raised with AST(SGOT)value more than ALT (SGPT)
value.

• Normal values of ALT (SGPT) is up to 45 U/L and of AST (SGOT) is up to 35 U/L

• When AST: ALT is more than 3 times, it is highly suggestive of ALD

• AST/ALT usually not more than 300 units/mL

• The AST and ALT values can also be normal in simple fatty liver or cirrhosis.

B)Gamma-glutamyl transferase (GGT) values in alcoholic hepatitis are usually raised above 100 U/mL (normal value is 0 to 30 U/L). Typically the GGT is raised in ALD but is less specific in advanced disease.

C) Bilirubin: the bilirubin value is normal (0.2 to 1.7 mg/dL) in simple fatty liver but can be high in advanced ALD. It is usually above 3mg/dL in alcoholic hepatitis.

 

2. CBC

  • MCV (Mean corpuscular volume) can be high.
  • High TLC and/or
  • Low platelet count can be commonly seen in alcoholic hepatitis

3. PT/INR

 

PT (Prothrombin time) is typically high in alcoholic hepatitis and advanced cirrhosis

 

4. Albumin

       Low Albumin level is seen in advanced liver disease.

 

5. Serum creatinine and blood urea

       High levels in a patient with liver disease can indicate the onset of renal dysfunction
